Treatment cost per patient diminishes the healthier individual participants are, if I remember correctly. The recent obesity epidemic and similar conditions relating to poor health could be related to recent healthcare price hikes. But like you said in your last paragraph the size of drug profit margins increasing and other factors could also apply.

I'm not blaming anyone but I do acknowledge the role the choices and decisions people make (and what types of food they can afford) can factor heavily into the cost of healthcare outside of socialism, capitalism and other systemic implementations. At the very least I think people should be aware of the degree to which the choices they make can have an affect.
